Report a Problem – Citizen Problem Reporter
4/28/2020- Citizens of Plainfield Township and the general public can now submit a problem to the Township 24 hours a day/7 days a week by using the online Citizen Problem Reporter solution. Citizen Problem Reporter is software that can be used by the general public to submit non-emergency problems or a request for municipal services in the community using a smartphone, tablet, and/or desktop computer.
It is the intent of Plainfield Township officials to utilize its existing Geographical Information Systems (GIS) software suite to better serve its citizens. Until this point, the process for the Township to address citizen complaints was: 1. Complaint is received, 2. Assign complaint to the responsible Township employee, and 3. Complaint is addressed. The citizen had no way of knowing the status of the problem without calling and talking directly to the Township Manager, a department supervisor or other administrative Township employees. The Township has now configured an ESRI GIS application to enhance this process entitled Citizen Problem Reporter, which will provide citizens the capability to not only submit their problems to the Township online, but to also keep track of the status of the problem as relevant Township employees review and take the necessary steps to address the problem.
The Citizen Problem Reporter application provides nine different categories problems can fall under, with different problems for citizens to choose from in each category. These categories of problems include the following: 1).Water/Sewer, 2). Health, 3). Blight, 4). Snow/Ice, 5). Park/Tree, 6). Trash, 7). Animal, 8). Land Use and 9). Road. The citizen selects the appropriate category, the type of problem, then enters their contact information and details about the problem they are reporting. The Township then assigns the reported problem to the appropriate Township department/personnel to be addressed in a timely manner.
This is an efficient supplemental enhancement to the Township’s existing day-to-day operations.
To begin using the Citizen Problem Reporter, click the below “Report a Problem” link button.
The following page is then displayed in a separate tab.
Once at the above page, click “Proceed as Guest”. The below map screen is then displayed. Continue with submitting the problem by clicking on the button graphic on the right hand of the screen that directly corresponds to the problem you are reporting (ex. Water/Sewer, Health, Snow/Ice, Blight, Park/Tree, Trash, Animal, Land Use and Road).

Once the appropriate problem is selected, the following page is displayed. Click the green “+Submit Problem” at the bottom of the screen.

Complete the below shown problem report form (including the “Type of Problem”, “Problem Details”, “Full Name”, “Phone Number”, “Email”, “Where is the problem located?” and “Would you like to add a photo?” fields) , click the “Report It” button, and then the problem will be addressed by the appropriate Township department/employee as determined to be necessary and appropriate.
